fix: update command names per plugin guidelines

Remove 'MCP Server' prefix from command display names to comply with
Obsidian plugin guidelines. Command IDs remain unchanged for API stability.

- Start MCP Server → Start server
- Stop MCP Server → Stop server
- Restart MCP Server → Restart server
