refactor: remove dead code from error-messages.ts

Remove unused permissionDenied() and formatError() methods that are never called in the codebase.

Coverage improvement:
- error-messages.ts: 82.6% → 100% statement coverage

This is Task 1 from the utils coverage completion plan.
